Post Title: Day and Night Time Support Workers Reports to: Senior Support Workers, Service Managers Main purpose of the role: The role of support worker is varied and this document is intended to describe the main functions of the role. This job description is not exhaustive and the support each service user will require will vary depending on individual need and ability. Applicants should consider this document as a guide to the role and tasks that may or will be required and they may be required to carry out, in a timely and efficient manner, any other reasonable task or duty that is requested. All support workers should be aware of the need to work to agreed care plans, as well as adhering to Aspire s policies and procedures. All employees have a duty of care to others under the Health and Safety at Work Act 1974. The role of support worker begins and ends with caring for people who are in need of care and support people who do not have a genuine interest in supporting and caring for vulnerable people cannot be considered as suitable for this role. Individual service users will have specific areas in which they require support which may include some or all of the duties outlined below. The main purpose of this role can be considered as follows: To provide a high standard of resident care in the home at night and undertake certain domestic duties which can be performed during night hours. To provide a safe and caring environment that promotes sleep and underpins the residents physical, emotional and psychological well-being. Key duties and responsibilities: To provide safe, efficient and effective care to service users: To help service users into bed where required. To carryout regular checks according to the individual needs of each service user as directed by the service management team. To respond to any service user during the night with all required needs including, personal care, company, reassurance and refreshment. To ensure the security of the premises following the departure of day staff. To ensure that service users are supported with personal hygiene needs ensuring their privacy and dignity of the person at all times. To ensure that any changes in need are reported and documented within the service users care plan. To ensure that the health and wellbeing of service users is monitored and to ensure that medical advice is sought should there be any form of health concern. To safeguard service users from harm and abuse and to report any concerns of abuse to the appropriate people either internally or externally. To support service users with continence management. To communicate effectively: To communicate with service user using their preferred method of communication To liaise in a professional with family members, social workers, health professionals and all other stakeholders involved with individual service users, sharing information that the service user has agreed to or is in their best interests, on a need to know basis. To handover any relevant information to senior support workers. To seek advice and report concerns to a line manager / manager on call as required. To complete daily log book, the communication log and any other documentation relevant to the service in a clear and concise manner, observing the Data Protection Act 1998 and Aspire s confidentiality policy. Other Responsibilities: To conform at all times with the Company s Health and Safety policies. To have knowledge of all procedures in the service. To safeguard and ensure the wellbeing of all service users, using the Company Whistle-Blowing Policy if required. To maintain confidentiality. To work as part of a team with day-staff members. To carry out light domestic duties as requested
